For the last two years, I've used the AT&T GSM conditional call forwarding rules to have calls to my iPhone forward to my office phone when  * Busy (*67*) * No Reply (*61*) * Not reachable (*62*)  Ever since I upgraded to the iPhone 4 on 6/24, the call forwarding fails and callers get the phone company's recorded message: "we're sorry your call did not go through. please try again later."  I've deactivated the call forwarding completely and reset each of the conditions. i've also verified with the status sequences that the call forwarding # is set correctly.
