Since there is no insurance like other phones for the iPhone and if I lose my iPhone how and will they replace or will I have to pay to get a whole new phone?

I am currently on a family plan with two other phones that are eligible for upgrades. Would I be able to transfer one of their upgrades to my line and be able to keep my current unlimited data plan as well? I would be upgrading from an iPhone 3G to a iPhone 4 if that matters at all.
Could someone explain why bluetooth doesn’t work like any other phone??? When I try to connect with another phone, it never works and I’ve tried it with my 3GS and now my iPhone 4. I’ve even tried to use it with my wife’s auto which has a bluetooth option and it never connects. It seems to be useless unless there’s something I’m doing wrong. I’ve tried all of other phones my family has with my wife’s vehicle and no problem at all, but not the iPhone.
I noticed that most iPhone users have phone cases. Presume this means the phone is very fragile. I have never used a case with other phones and never encountered a problem. Should I get the AT&T offer Loss Protection Insurance for the iPhone 3GS in my household to protect against loss or damage? If so, how much is it?
